• Usage data. Information about how you access and use our website, including IP address, browser type, pages visited, and time spent on pages.

Census Home does not request or accept protected health information. Please do not submit patient, client, or care-recipient names, diagnoses, medications, insurance IDs, or clinical notes through our forms. Census Home is not a healthcare provider and does not determine care or benefit eligibility.

Opt-in occurs only when you check the appropriate unchecked checkbox on the form. Marketing messages (such as related offers) and non-marketing messages (such as application and scheduling confirmations, assessment reminders, and follow-up about your inquiry) are opted into separately.
